ON-LINE LICENSE RENEWAL

Please read the instructions below carefully before logging in to Renew Online.

  1. The renewal process should take between 10 and 15 minutes to complete. If you do not complete the renewal application in one session, your data will not be saved.

  2. Review your Physician Assistant Profile before starting process. To view your profile return to home page click on “Licensee Search” – click on the Physician Assistant circle. You will then be able to enter your name or scroll down to enter your Arizona license number. If changes to your prescribing authority listed on your profile are required, please have your Supervising Physician submit a completed Prescribing Authority Modification form to the Board. If a Supervising Physician is listed on your profile and should be removed, please submit the Prescribing Authority Modification Form with the appropriate box checked to indicate withdraw of prescribing authority modification. Submit to the Board by fax (480)-551-2704 or mail.

  3. At the time of renewal, physician assistants must provide a completed CME attestation form and a narrative regarding any “yes” answer(s). If you do not have the required amount of CME you may request a 30 day extension in writing prior to your license expiration date. At the time of renewal all files are reviewed for completeness and if it is determined that additional information is needed, a deficiency letter will be issued. Licensees may provide documentation to the Board via licensingreport@azmd.gov or via fax or mail. If required documentation is not provided to the Board within 60 days of the deficiency notice the license will expire on its current scheduled expiration date.

  4. Payment Methods: Visa, MasterCard and AMEX are accepted.

  5. All licensed PAs with a DEA Registration or who intend to apply for a DEA Registration in Arizona are required to register with the Controlled Substance Prescription Monitoring Program.
