Frequently Asked Questions

How do I request information from the Board?
The Board must receive a request for public records. Requests may be made verbally or in writing, however in order to expedite your request a written request is preferred. For your convenience, there is a Public Information Request form on the Board’s website. The Board will gather the requested information and invoice you for the cost of the copies. Upon receipt of payment, the information will be mailed or faxed to you or will be available for pick-up at the Board’s office. Public information requests are processed within a reasonable timeframe.

Is there a charge to obtain public information?
All information located on the Board’s website, www.azmd.gov, is available at no charge. Additionally, you may view copies of the public information at the Board’s office at no charge. If you would like copies of public information sent to you, the cost is $1.00 for the first three pages and $0.25 for each additional page. For your convenience, a public information request form may be downloaded from the website.

What is considered to be public information?
License file information, Board orders (i.e. Letter of Reprimand, Decree of Censure, Probation, etc.), dismissal letters, advisory letters, Board meeting agendas and minutes are provided upon written request. Additionally, transcripts from formal interviews and formal hearings are available through Ottmar & Associates at (602) 485-1488.

What information is available on the Board’s website?
Almost all of the Board’s public information on a physician is available on its website. Under the Physician Profile section, you can find the physician’s name, license number, license status, license renewal date, address, and phone number. Other information includes educational background including residencies, internships and fellowships, all disciplinary actions.

What information can I obtain that isn’t on the Board’s website?
Copies of license applications and pertinent licensing information, dismissal letters, and advisory letters are available on written request. However, dismissals and advisory letters do not contain any additional case details.
